A follow-up cxr showed right hilar density had persisted though the pneumonitis had cleared. first radiologist reported lymphadenopathy as likely however the second radiologist suggested it could be normal vascular structures. do i need a ct?

That decision should be made by someone who knows your full history, exam and testing along with why the study was done to start with. Sometimes another chest x-ray at a different angle will clarify the issue without the additional radiation and expense of a CT.
