Hello, 42 yr old male, controlled hypertension, recent lipid profile is: total cholesterol:183 hdl: 39 ldl: 119 triglycerides: 131 nonhdl:144 & a1c 4.9 creatine: 1.36 my doctor didn't recommend a statin, but wants a renal doppler ultrasound. advice?

Thank you for your question. Your cholesterol numbers aren’t very terrible it’s not time yet to start a Statin but I won’t be surprised if we have time you may be put on one in the future. Your creatinine clearance is a little high so maybe they’re concerned about your kidneys however I am not convinced that doppler studies will find a problem with the kidneys. weight loss and Exercise Will help

Serum creatinine is mildly elevated but your kidney function is not that bad if you are black or your BSA above 2.0. Yes, renal ultrasound with or without renal artery flow check is the standard of care for a patient with hypertension and abnormal kidney function. Regarding your lipid profile, more exercise to increase HDL above 40 may keep you off from statin in the near future.
