All in the numbers.: The thick scab is more than likely a callus caused by frictional pressure between the fifth (little) and the fourth toes. Normally, the lesser toes have three bones each. On occasion the little toe has only two. When this happens, pressure from the toes bones on opposing toes causes a callus. This can be very painful, aggravated by shoes and verified by x-ray. There are several simple solutions.
