Not much: Your triglycerides maybe up a bit from the grease that helps make donuts so much fun to eat. If your physician is of the school that thinks this actually matters, let him/her know you cheated. It may raise your glucose slightly and if this is about diabetes and was supposed to be fasting, again confess your donut. There's no other real impact. Eat sensibly overall, stay fit, and enjoy donuts.
