Nutrition & exercise: There's no secret to healthy nutrition & regular exercise to improve your health, especially heart-wise. Eat lots of brightly or darkly colored fruits & vegetables. Eat whole grains w/fiber. Eat healthy fats like wild caught fatty fish, olive oil, nuts & avocados. Eat lean protein like beans, eggs (whites especially), wild caught fatty fish & chicken. Aerobic exercise is good but so is strength.

At this point no: If your relative is having chest pains and an abnormal ekg suggestive of a blocked artery, the only way to improve blood flow is to open up the blocked artery either with a stent or surgery. Doing strenuous exercise if he has a blocked artery can be dangerous and can lead to a heart attack. It is never too late to start eating healthy by avoiding fatty foods and simple sugars.
