Work with trainer: Having someone to help you personally can make a huge difference both in exercise technique and accountability. You could work with a good trainer and nutritionist in developing a program that will work for you. Diet is extremely important. You need to avoid processed, sweet foods and replace them with fresh veggies, fruits, lean proteins, low-glycemic carbs, and healthy fats, etc.
