Depends: Scoliosis newly seen in adults unusual & first need to discover if fixed (organic) or functional (change with position). In persons with unequal legs or loose & unstable pelvis flexible spine curves when upright under impress of body weight (gravity). Functional scoliosis common in fibromyalgia. Weight loss, back exercises, yoga, & aquatherapy helpful to enable muscular development & compensation.

varies: It varies based upon the size of the curvature and the degree of symptoms. From purely a structural perspective, curvatures less than 45 degrees usually dont progress, but larger ones do. So, for example, in the case of an adult with a 70 degree curve, surgery is the only option that can keep a curve from progressing. From a symptomatic perspective, there are many non operative things that help.
