My grandpas creatinine levels went from 4.75 to 4, is this still very high? they have been keeping him in the hospital to monitor?

Yes, it is still significantly high especially if he has acute kidney injury, normal serum creatinine is less than 1.2 mg/dl. However, there is improvement and if creatinine drops again the next day, his kidneys are recovering and he can be discharged if kidney problem is the only thing which keeps him in house.

Yes, it is still high. While there is still a good possibility that kidney function will improve to baseline values prior to the acute event, follow up with a nephrologist will be needed as outpatient.
