Ent suggested i use nasal irrigation to help with eustachian tube dysfunction and now feels like fluid is stuck behind eardrum with tremendous pain and pressure. eardrum has small perforation but pressure pain remains. is there a way to clear it?

The saline solution could not reach to your middle ear but the obstructed eustachian tube is the reason for the fluid building up in your middle ear which is called otitis media. When the fluid is big enough, it will drain to your outer ear. You can try Nasonex or Nasacort to reduce the nasal inflammation which can open the eustachian tube. When it opens, fluid will stop building up in middle ear.
