A tall peaked P wave on an EKG , called P pulmohale, may be a sign of enlargement of the right atrium of the heart which can occur with elevated pressure in the pulmonary artery as well as in chronic lung disease. I have also seen tall peaked P waves transiently appear during rapid heart rates. An echocardiogram will be able to diagnose right atrial enlargement as well as pulmonary hypertension.
