Both are measures of inflammation in the body. To my dismay some hospital labs are no longer performing esr which I have found more helpful in following patients with such diseases such as polymyalgia rheumatica in titrating their prednisone therapy.

Both are markers of inflammation but are not interchangeable. ESR is more often positive in patients with inflammation of bone and skin than CRP. It is recommended that both tests be done at the outset and course can be followed with either, if both are positive, or by using the test that happens to be positive.
