Pelvic u/s showed 4.6x4.7 mass with numerous echoes in left adnexal region. what does that mean exactly? dr adamantly recommended biopsy. cancer?

Not necessarily but-: Adenexal masses are common in women. Most of them are benign cysts. But if they are complex cysts like yours appears to be, then further evaluation including biopsy/removal of this mass should be considered. I would advise you to follow your Gynecologist's recommendations.
