Many things!: Most common cause of enlarged cup to disk ratio, "enlarged optic nerve", would be glaucoma. Other causes could be; physilogic (you were born that way and it is genetic, and of no significance), tumor, compression of the optic nerve by something (a mass), nutritional.

Your eye doctor may have identified nerve swelling termed papilledema, which is due to increased pressure of the cerebrospinal fluid. Causes for this include, obstruction, tumors, vascular problems and trauma. The neurologist will know which tests to run to discover the cause.
