Depends: It most likely referring to optic nerve Cup to Disc ratio that is an important eye exam assessment for glaucoma. The ratio needs to be smaller for healthy nerve if its larger or asymmetrical then it requires follow up for glaucoma. Eye pressure 17-20 could be high or normal which depends on the patient's history, optic nerve condition and risk for glaucoma.

Its a risk factor: Asymmetry of the optic nerve between eyes is one known red flag for potential glaucoma. Also a slightly elevated eye pressure in the eye with more "cupping" can be another risk factor. It will be best to have each eye evaluated in an exam with a visual field test and possibly photos of the nerve to evaluate how healthy it is. It made need treatment or just close observation.
