Titrate down: I rec speaking with your doc & titrating down to a dose your comfortable with prior to changing medications.

Acetyl l carnitine: Acetyl L-carnitine seems to be beneficial for symptoms of diabetic neuropathy. If your neuropathy is from something else, it might not be as effective. Nortriptyline and venlafaxine are also alternatives for neuropathic pain, as is Gabapentin or pregabalin.

Different Class Rx: You had a snri others in this class are effexor, and the triptyline-like drugs which probably won't work without the same problems.The anti-seizure meds like lyrica, (pregabalin) gabapentin, sometimes work for severe symptoms.Local anesthetics like topical Lidocaine or Mexitil an oral form sometimes help finally, if no relief with these meds an opiod like morphie may be necessary. Spinal stimulator last resort.
