Yes: A ruptured silicone breast implant can, not alwys lead to encapsulation and hardening of the breast. A ruptured silicone implant always needs to be exchanged.

It can lead to : Caspular contracture which is a hardening of the tissue around the implant. Capsular contracture can cause distortion of the breast. The older generation implants tend to leak more than the newer ones if the shell is broken. Older model implants can cause more distortion.

Cohesive gel: Cohesive gel implants are being used today for breast augmentation. A cohesive gel tends to stay together and limit gel movement away from the implant pocket, . An implant that has excessive scar tissue around it will look round and feel hard. The studies on cohesive gel implants show less severe deformities.

Possibly.: The currently utilized silicone gel breast implants may leak and not cause any distortion at all. Sometimes, leakage may not be detected clinically and MRI studies may be necessary to detect this leakage. At other times, leakage may result in a visible change and/or hardening of the breast ( breast implant encapsulation), secondary to scar tissue forming around the leaked implant.

Extent of the leak?: The amount of deformity is proportional to the extent of the leak and involvement of surrounding tissues. Generally speaking this should be removed at the time of implant extraction to the best of the experienced surgeon's abilities. Silicone can rupture within the capsule and be contained with minimmal deformity or can rupture outside the capsule with extensive involvement of neighboring structu.
