Yes: If there is an abscess, especially with significant cellulitis, you will need both surgery to drain the infected source as well as antibiotics to treat any residual soft tissue infection.

Yes: Absolutely! if you have a perirectal abscess which is infected and painful, you need antibiotics and drainage in order for it to resolve.
