Yes: Many agencies have social workers who are out in the field and go into the person's home depending on need and mission.

It depends: I'm not sure why you're asking. If you need a social worker to do a home visit hopefully you can find one willing to come, though many places don't have enough to meet everyone's needs. If you're asking because you do not wish them to intrude uninvited, while I'm not a lawyer I believe you don't have to let anyone in your home unless they have a court order/warrant, in which case you must comply.
