Being Prepared: Most of the time they are looking to see if you have discussed your wishes with your family about what you want should the time come to need a nursing home. This is the perfect time to discuss it- before you actually might need it to find out what you are thinking and what your family is thinking.

To give you options: Some nursing homes have waiting lists or do not accept your insurance, so a SW explores your options while you and the doctor decide what care you will need and if that can be be in at home or otherwise. If there is a waiting list, then getting your name on the list allows you to chose that facility when available. Most seniors prefer to return home but this is not always possible or safe.
