Yes: Flat feet, pes planus, can contribute to low back, hip and knee pain as can over pronation or over supination of the feet. The feet provide the base for the body. If there is something out of alignment or not providing the "shock absorbing" that is needed it is transmitted up the "kinetic chain" of the body. Flat feet lack the arches that provide the "spring in the step" that absorb the shock.

Yes: Yes, the feet, knees, hips and back are all interconnected and can be a cause of pain relative to another. You should see an orthopaedic surgeon to do a complete history and physical.

Yes it can: Definately. Poor biomechanics in the feet which are the foundation of the skeleton will cause biomechanical faults moving more proximally (higher up).
