Depends: Could be several things. Sleep Apnea is high on the list and can be diagnosed (or ruled out) with a simple sleep study. Another possibility is a primary cardiac arrhythmia and that can be diagnosed with a 24 hour heart recording. It's also possible that you have primary sleep disturbance with nightmares such that the heart pounding is secondary to and appropriate for what's happening.
