If abscessed or Infx: If severely infected, a tooth in a 3 year old may in a rare case need extraction, etc.

Yes: While we would prefer to save the tooth, if possible, sometimes this can not be done if the tooth is to badly damaged. In that case an extraction would be necessary. A badly decayed baby tooth should not be left in place as it can lead to infections that can damage unerupted adult teeth and can even cause death if left untreated. Please have your child see a pediatric dentist asap.

Of course: The main reasons for having a tooth removed at any age would be a dental emergency that has caused trauma, infection or disease, and advanced tooth decay.
