Waste: For most people the otc appliances are a waste of time and money. Know that night guards, to be effective, must be exquisitely adjusted. Also know that night guard is a band-aid, treating TMJ symptoms, but not the cause. You need to bite the bullet and have a new night guard fabricated, and you need to see an orthodontist to see if malocclusion is causing or contributing to your tmj.

OTC products: It is unfortunate that you have had a problem with your current dentist. Chances are very good he/she has discarded the models.. If we kept every model of every patient we would need a warehouse! store bought guards do not fit well and will not be comfortable. Insurance or not, prioritize your funds and find a new dentist who you like to help you with your bite problems.

NO OTC: An otc mouthpiece for tmj/tmd is the last consideration for helping your tmj. Depending upon how old your other mouthpiece was, the dentist may consider some type of discount. Don't go too long without.
