If : If i understand correctly, you have had 2 infections involving your right breast?  Were the implants removed each time?  If you had an infection that involved the implant, you will need to have the implant removed and left out for a period of time to allow the tissues to heal.  I would normally wait at least 3 months before considering replacing an implant after an infection.  I would really need more information to give you any detailed advice.  However, there is really no reason why you should not be able to have a successful outcome but it may take time/.

Yes: Muscles need no approximation or repair in surgery as they heal with time.

No: The MRI is the gold standard for evaluating gel breast implants for a possible rupture but are expensive and may not be covered by insurance. A less expensive way which might give the information would be an ultrasound.
In any case you should see a board certified plastic surgeon to have this evaluated.
Also, are you up to date on your mammograms?

See your surgeon: You definitely want to see your surgeon. Could be a lot of things including a nerve that is sending inappropriate messages, scarring, infection, or rupture.

Breast Implant Leak: As with any prosthetic devise, proper protection and care are necessary.  Saline implants have a small percent of rupture which increases with the time that the implant is in place.  Trauma from fall, breast or chest wall impact or from sudden deceleration due to air bags or seat belts have been reported.  The newer silicone implants have semi-solid gel and are less likely to leak due to trauma.

in 2019 i got cohesive gel silicon breast implants. i was told to get an mri every 2-3 yrs. i am scared to do so plus it is costly. will an ultrasound reveal any tears or ruptures on the implant?

: It might, but spontaneous tears or ruptures are unusual this soon and are not usually the reason the MRI is recommended. You need to clarify with your doctors why they are recommending this test.

can breast implants cause neck rash?

No: In the absence of some severe infection which would have obvious symptoms on your breasts, breast implants will not cause a rash on the neck.  Silicone is a very inert material and allergic reactions to it are essentially non-existant.
