Many possibilities: Could be an infection, gout, a response to an earlier trauma. He needs to be seen by a physician asap.

In the meantime, if it is usually safe for him, Ibuprofen can help (800 mg, or 4 of the otc pills) three times a day while you await being seen by the doctor.

Get in to see doctor: He may have an infection so he should be seen immediately. It may also be some form of arthritis but infection needs to be considered first.
