You mean fluid?: Most of the time fluid behind your eardrum is a result of congestion in your nasopharynx such as from a cold or allergies. Any reason that's causing the closure of the eustachian tube will ultimately result in a watery fluid buildup in the middle ear space. Decongesting this area requires diligent use of topical decongestants. I have my patients use a mixture of oxymetazoline and alkylol.

Need more info: And an exam would help. If your doctor feels that tympanotomy (hole in ear drum) is necessary and you disagree or are in question, then get a second opinion from an ENT specialist who can examine you and obtain a full history. Good luck.
