If amoxicillin is working on a tooth does the pain get worse at first?

Possibly: Regardless of whether it is "working" or not, tooth pain can continue to increase even after oral antibiotics.

It may: Sometimes it can. It depends on the how fast the bacteria are growing at the time you begin taking amoxicillin. However, the Amoxicillin is working at killing the bacteria and will reduce pain.
