Certainly More: Dysphoria refers to low mood. You are describing symptoms concerning for overt major depression. Can you tell me what cbt is?

Options: Hi. Talk to your primary care doctor about other organic causes for your depression. Don't give up on treatment. Even if your antidepressant and cbt for two years seem to have made it worse, there are still other options (other kinds of therapy, other medication). Do talk to your therapist about it, you should not be feeling like your treatment is hopeless, even if it is difficult at times.

Treatment results: If you're continuing to feel this way after 2 years of treatment, it's time for a thorough review of both your clinical situation and response. Something more may well be going on, and it needs to be attended to. You may want a second opinion on your case -- also considering bipolar, ptsd, personality disorders, and other medical conditions which mirror and/or worsen depression. Be well.
