Many reasons: Three or more episodes of : tonsillitis per year, recurrent fever above 101 degrees f, recurrent purulent material on the tonsils, recurrent positive strep throat culture, enlarged tonsils causing upper airway obstruction, dental malocclusion, sleep disorders or difficult swallowing (dysphagia) recurrent abcesses of tonsils (peritonsillar abscess).

Guidelines 2011: Sleep apnea is by far most common reason, 7 streps in 1 year, 5 streps per year in 2 years, 3 streps per year in 3 years, chronic tonsillitis/sore throat, concern for mass.
