Many possibilities: Hard to say just from your description but some answers would be periodontal disease, bruxism(teeth grinding), overly agressive tooth brushing are some examples. Either a consult with a general dentist or a periodontist would be a good idea.

Dental concern: Due to the symptoms you've described, i think it's time to see your dentist. You may have excessive bony growth, but the wearing of enamel should be corrected.
