What causes sporadic, sharp, needle like pain on top of great toe? feels like a bug bite.

Toe pain: Could be a pinched nerve or a nail problem. If you can reproduce symptoms by pushing in and around the area, then the problem is local and a podiatrist or foot doctor could help. If you that way then it could be a pinched nerve at the back. In this case you would be best served by seeing a pm&r doc or neurologist.

Assuming : There is no bug bite as likely if her was you would see some sort of redness or skin irritation, then it is possible there is a nerve compression which could be local like the deep peroneal nerve by the first metatarsal base or possibly from the lower back.
