MANY things.: Generally when someone describes their pain the way you did, it's some type of nerve irritation. A small nerve may be running over a bony prominence and be irritated by your shoe gear, or may be caught up in other tissue. See a podiatrist for a more detailed exam and diagnosis. Treatment may be very simple!

A few things: Sharp pains in the toes can be due to arthritis, nerve irritation, injury, poor fitting shoes, diabetes, poor circulation, thyroid issues among many other things. You may need to see a podiatrist and get x-rays to narrow down the possibilities and start treatment.
