Surgery: You can look at surgery to see how bad the tubes look. Ideally they should be removed before ivf to improve outcome. Sometimes a tube can be opened up. That does not mean it will work but it gives you a chance.

Surgery poor choice: Surgery for hydrosalpinx has a low success rate. The delivery rate is commonly about 20% over 12-24 months whereas, ivf can be as high as 60-70% from a single cycle. Tubal occlusion is as effective as removing the tubes. I have seen some women have a serious drop in egg number after removal of tubes. You do have a choice but often not a good one. Ivf is safe too.
