Shouldn't : Your eyelids should appear smoother or smaller with the extra skin (and possibly fat) removed. If they look the same to you, ask your doctor to see your before pictures.

Gravity!: It is possible that after several years gravity takes its toll. Repeat blepharoplaty my be needed. You will be better off with a conservative doctor rather than an aggressive one that takes off too much (which leads to dry eyes... Misery).
