You should have a kidney/ureter/bladder ultrasound to look for renal stones. Renal stones can be non-moving and thus, they don’t cause the typical pain. But, they can get infected and result in problems like this. The other possibility is that the bacteria are only partially treated with the antibiotic given-you should treat it with a different antibiotic and repeat the culture on the antibiotic.

There should have been a sensitivity report and treatment based on it. In the meantime drink water, some cranberry juice, keep the area clean with soap and water and repeat a clean catch or catheter specimen of urine for reC&S, Good luck
