Hospitalized for high hr 6 months and told it was svt. had echo with bubble study and numerous ekgs since and told everything is great. i've notice on my hr watch 5 spikes during sleep since. one as high as 208 but never on holter. major concern?

You had a thorough cardiac work-up recently and you were completely asymptomatic during those "spike" episodes so I do not think you should concern about those recordings. If I was you, I would stop wearing that smart watch and only re-wear it when I have cardiac symptoms such as palpitation, shortness of breath, etc. just for a correlation btwn heart rate & symptoms, not for detecting arrhythmia.
