I am 41 y. old, chronic itp since 2010, removed spleen 10 y. ago, doctors used to treat me by prednisolone 20 mg mycophenolate mofitil 1000 mg cyclosporine 100 mgazathioprine 100 mg till now but all in vain blood platelets didn't exceed 10.000 ?

Yes, definitely of concern for refractory ITP. Your hematologist remains your best resource as there are newer drugs approved for treatment of ITP, further workup might be needed including bone marrow evaluation.
