My baseline resting home bp with valsartan 40mg is around 108/64. should i ask for a lower dose? not sure if this is the minimum effective dose. should my goal be under 130/80 or 120/80 or 110/80? in dr's office bp spikes much higher. using cpap?

At your age, this blood pressure is good as long as you are asymptomatic. You should continue with current valsartan dose of 40 mg, which is the lowest dose. Home blood pressure is most often lower than office blood pressure; however, I still want to have your manometer checked by your provider (preferably with a reliable, manual sphygmomanometer) to make sure its accuracy. Your BP seems to be at goal, you do not have to ask your doc to make changes, let the doc discuss options based on current readings. You likely have a component of white coat hypertension and using CPAP consistently will help with managing hypertension in the long run.
