Bilateral pulsatile headache on top of the head/temples + pulsatile tinnitus when physical effort during simple daily activities and when lie down. light+noise sensitivity. spinal tap, brain mr normal. neck, heart ultrason normal. what are we missing?

You have had a lot of testing for what seems to be simply a complex migraine issue. See a headache specialist, as there are multiple interventions that can help. At the very least, the headache and light/ noise sensitivity can be abated.

Brief episodes of pulsatile tinnitus left ear only upon standing up? . Multiple normal 3t mri/a cta head neck for headaches. Normal BP and weigh.
Had hiccups lasting less under 5 mins, during which rt ear first made a tone, felt full then there was a pounding/heart beat sound for 1-2 mins. Have gotten ear thing in past but rarely 1-2 year. Worry some? Is it pulsatile tinnitus? Normal BP 105/65?
