The white spots are likely just changes in his circulation in that area that revert to normal. The human body has the capacity to shift superficial blood flow which supports our usual skin tone to deep vessels, as a protective device. Skin will turn pale when it happens. It is not what I consider a worrisome sign. And no, you don't have CF at 36 with nothing medically bothering you.

Definitely worth having your son examined. It is not likely that he has cystic fibrosis. However he likely has anxiety/panic disorder exacerbated by Internet indiscriminate use and that will eventually interfere with his ability to complete his activities of daily living.
