I recieved cohesive gel silicone breast implants in 2019 and told to get an mri every couple yrs. can i instead get an ultrasound to detect any punctures or tears?

An ultrasound is not an appropriate or adequate substitute for an MRI, nor is the likely principle concern detection of punctures or tears. Since this is not a routine recommendation after breast implants, it is likely your doctors have additional concerns and therefore want more careful follow up. You should discuss that with them.
