Psa 3.7. last year at this time was .9. inconsistent right hip and back aches. left flank aches that are bit more constant. biopsy has been done. likely stage 4? 52 years old?

I doubt stage 4 with a PSA of 3.7 and your hip and back pain is less likely due to metastatic lesions. If biopsy is positive for prostate cancer urologist will order bone scan for ruling out metastasis. Good luck.
