If you are chronically symptomatic and problem has not improved with aggressive physical therapy +/- local therapy, you need to see a spine orthopedist for laminectomy or a minimal invasive spine surgery with vertiflex procedure for the described LS spine changes.

Do recommend a visit to a spinal surgeon for a consultation, as surgery may be an option. Conservative interventions include the use of a sacroiliac belt, physical therapy, ongoing back exercises, swimming, and even periodic acupuncture.
