37 yo w/ chronic pelvic pain.cystoscopy&psa normal.mri results: "mildly enlarged prostate w/ heterogeneity of the signal of the peripheral zone. recommend dedicated multi parametric mr imaging of the prostate for further characterization. meaning?

Peripheral zone is not of uniform configuration. Multiparametric MRI gives many more views of the area. Chances are that it is a variation of normal or prostate enlargement of the area and not related to his pelvic pain since all other tests are OK

38y.o. man w/ mildly enlarged prostate noted incidentally on pelvic mri. Path report recommended more specific prostate mri but doc thought unnecessary due to psa being wnl (.9 psa & 32.2% free psa). Worrisome for cancer? Push for mri or no big deal?
What do you recommend for persistently elevated PSA (8-9) with multiple negative prostate biopsies and negative prostate MRI?
