My psa has been fluctuating from 5.7 ( 2018 ) to 4.0 ( 2019) to 5.1 (2020) to 6.1(2023). my free psa is currently 30% for my psa reading of 6.1. my prostate size is 84cc. i am 62 years old. i had a negative biopsy in 2018. do i need to get a mri ?

There's still a 25 % chance that you have prostate cancer if prostate infection or inflammation aren't responsible for your PSA fluctuations. An endorectal MRI would be a reasonable test for you if your urologist agrees.

A negative biopsy and a free PSA are a good sign that chances of malignancy are low. A decision for MRI should be made in consultation with your Urologist as even that is not a 100% test. Choices of follow up or tests and re-biopsy will depend on your family history, exam, your degree of your concern and your doc's recommendation. Good luck
