35 year old male 235 lbs. been taking 100mg of losartan once a day. after about 3 weeks on the 100 mg (previously taking 50 for year), no change (e.g. 145/101 range) dr. added metoprolol 50mg. heart rate was also tachycardia at rest. is this too much?

I have a couple suggestions: try basic principles: 5% weight loss + low salt diet & change Losartan to a new ARB such as Telmisartan 80 mg, day for 2 months. If BP is not less than 130/85, a non dihydropyridine calcium channel blocker (verapamil or diltiazem) should be added instead metoprolol because you are young for taking a beta blocker due to side effects (sexual dysfunction and depression).

It's best to increase blood pressure medications until the pressure is normal. In addition, it's important to search for underlying causes of persistent hypertension. In rare instances, a tumor secretes substances which raise blood pressure and increase heart rate. An astute cardiologist, internist, endocrinologist or primary care doctor can arrange for appropriate tests for these rare problems
