This patient likely has uncontrolled hypertension for a long time. I would guess he had cardiac work-up due to dyspnea on exertion and possible orthopnea. EF looks good by number but it does not because of grade 3 mitral regurgitation. He should be referred to a cardiac surgeon for mitral valve repair or replacement, I think.
