Transvaginal ultrasound did not reveal apparent lesion such as fibroid or polyp or thick endometrium which can explain for the heavy bleeding but the presence of scattered areas of increased vascular activity within the endometrium (scatterred areas of vascularity) deserve a direct exam by hysteroscopy +/- endometrial biopsy. Remember the cancerous tissue is usually abundantly supplied with blood.
