My kappa flc count is a 2.17 mg/dl (range on test is .33 - 1.94 mg/dl) and my lambda count is 1.31 mg/dl (range is .57 - 2.63 mg/dl) ratio is 1.66 (range .26 - 1.65) spep is normal? should i be worried about ratio being .01 outside of normal range?

Although the ratio is only .01 above the upper limit of normal but serum kappa FLC is high so the result has to be interpreted cautiously. I wonder if you have 24 hour urine collection and or urine immunofixation. If kappa light chain is high in the urine, kappa light chain disease may be present. I assume your kidney function is still normal because those light chains are much higher in CKD.

No. A kappa/lambda ratio is of limited usefulness. A normal ratio does not exclude disease and an abnormal ratio is not diagnostic of anything. Your results are not a cause for any concern. Wish you good health!
