Not feasible to be precise, based upon your post, but might consider the following: not clear, so far, why you have lengthy myelomalacia, but this could be secondary to Neuromyelitis Optica, Spinocerebellar Degeneration, etc. Has effort to uncover etiology been accomplished? Have you had full evaluation from an experienced neurologist? Lastly, not clear as to what question needs to be answered.
